You have to look at your information the way Amazon looks at it.
Your side of the invoice should match as closely to your Amazon account as possible. It should be made out to Amazon account name or the name of the Amazon account owner. I know this sounds ridiculous. The email address for you should match the email address on the account that Amazon has for you. Everything should match as much as possible.
